Personal Health Plans will be used by Health Trainers to identify goals for people who want to live a healthier lifestyle.
People across Hull will work with a Health Trainer to identify personal goals that can help improve an individual’s health. This could include stopping smoking, eating healthily or increasing levels of physical activity.
The team of Health Trainers will also use this health plan to identify other health needs and possibly refer to other services that can provide specialist help and support.
